MUTUAL AGREEMENT TO MAINTAIN PRIVACY
Dr. Leon Tcheupdjian, his associates, The Liposuction And Cosmetic Surgery Institute and Longevity Spa (heretofore referred to as The Institute and Spa), and the staff agree to maintain privacy policy as outlined in the HIPAA from. Furthermore, The Institute extends greater degree of privacy than is required by HIPAA, state of Illinois confidentiality mandates, and common law. Non of your information will be disclosed to anyone for any reason without your personal, prior approval.
Our facilities are your ultimate relaxation destination so please turn off your cell phones during your visit. Recording staff conversation taking photo or video of staff or premises is absolutely prohibited with no exceptions.
In consideration for services provided, Patients agrees to refrain from directly or indirectly publishing or airing derogatory commentary upon Lipodoc.com, Liposuction and Cosmetic surgery Institute Dr. Leon Forrester Tcheupdjian MD, or any of the staff or his associates, The Institute and Spa, and the staff expertise and/or treatment unless mandated by law. Publishing is intended to include attribution by name, pseudonym, or anonymously. In Addition, Patient will not denigrate, defame, disparage, or cast aspersions upon Dr. Tcheupdjian, his associates, The Institute and Spa, and the staff and will use reasonable efforts to prevent any member of their immediate family or acquaintance from engaging in such activity. We consider derogatory commentary, including but not limited to message board posts, blogs, radio show interviews, television interviews, pamphlets and print media interviews, a character assassination. We will spare no resources in litigation in response to any of these attacks on our character(s).
In turn, Dr. Tcheupdjian, his associates, and The Institute and Spa, and the staff agree not to provide any information for marketing or be compensated for forwarding patient lists or protected health information to any entity via third party marketing.
Both Patient and Physician will work to prevent the publishing or airing of commentary about the other party via internet blogs or other electronic, print, or broadcast media without prior written consent. Finally, this Agreement shall be in force and enforceable (and fully survive) for a period of the longer of (a) five years from the last date of service to Patient, or (b) three years beyond termination of the physician-patient relationship.
Patient and Physician both acknowledge that breach of this Agreement may result in serious and irreparable harm. In addition to compensation for consequential damages, Patient and Physician agree to the right of equitable relief (including but not limited to injunctive relief). Should a breach of this Agreement result in litigation, the prevailing party shall be entitled to reasonable costs, expenses, and any associated attorney fees.
Patient are given the opportunity to have all questions asked and answered.
General
By using this Web site, you acknowledge and agree that these Terms and Conditions, which include our Privacy Policy constitute the complete and exclusive agreement between us concerning your use of our Web site, and supersede and govern all prior communications, agreements, or other proposals.
