When Are Breasts Considered “Sagging?”
Breasts are considered to be “sagging” if one or both of the following scenarios are present:

The nipple is pointing downward instead of forward. In more extreme cases, the nipple may appear to be hanging from the bottom of the breast.

The nipple is located below the inframammary fold. The inframammary fold is the crease that is located between the lower breast tissue and the ribcage. You can estimate whether or not your breasts are “saggy” by using the pencil test.
What is the pencil test? The pencil test involves placing a pencil against the inframammary fold, parallel to the floor. If the nipple sits below the level of the pencil, then we consider the breast to be saggy. Typically, a patient will require mastopexy to correct this.
