Terms and Conditions
Please read these terms and conditions carefully before scheduling an appointment with our practice and/or ordering any of our cosmeceuticals and/or our supplements. All users agree to abide by these terms and conditions. If you do not agree to any of these terms and conditions, do not schedule an appointment with our office and please do not order anything from this site.
Appointment Scheduling
All appointments scheduled with Forrester Clinic, DBA Liposuction and Cosmetic Surgery Institute, Longevity Spa, Wrinkle Fairy, and any other affiliates are times set aside exclusively for you. Our medical providers maintain robust schedules and their time is valuable. With that said, if you schedule an appointment for any non-surgical service, understand that canceling this appointment within 24 hours of the scheduled appointment time may carry a non-refundable penalty of $25, at the discretion of management. The decision to charge this $25 cancellation penalty is final and cannot be appealed. Additionally, a surgical procedure that requires any kind of anesthesia requires us to assemble a surgical team that includes anesthesia staff, surgical technicians, PACU RNs, and the surgeon. In order to assemble this medical team, a non-refundable deposit of at least $1000 is required to reserve your operative room time.
Shipping
At present, we ship to only the United States. If you live outside the United States and wish to order any of our products, please contact us directly at +01-847-259-1000, and we will do our best to fulfill your request at a mutually agreed upon shipping fee.
If you enter your address incorrectly, and our shipping software fails to correct it, you will be responsible for any reshipment costs once the item is returned to us.
Delays caused by the shipping company are out of our control. Any delay in shipping as a result of failures internal to the shipping company will not result in any consideration and/or refunds. If you provide an email address at the time of order, you will receive email updates when your package ships, should it meets a delivery exception and when it’s delivered.
Refunds
It is our policy that all sales are final, and no refunds will be issued at any time. Additionally, all $25 charges for cancelling or no-showing for an appointment within 24 hours of a scheduled appointment time are non-refundable for any reason.
Limitation of Liability
Forrester Clinic, DBA Liposuction and Cosmetic Surgery Institute, Longevity Spa, and any other affiliates, shall not be liable for any special, adverse or consequential damages that result from the use or the inability to use any of the products for sale on our website. If you are pregnant, or may be pregnant, or are currently breastfeeding, we advise you to refrain from using any of the products for sale on this site.
Forrester Clinic, DBA Liposuction and Cosmetic Surgery Institute, Longevity Spa, and any other affiliates, make no claims or promises based on the effectiveness, effects and/or benefits of any product for sale on this website. All supplements for sale on this site have been evaluated and approved for consumption (based on the instructions listed on the packaging) by the Food and Drug Administration (FDA). However, the FDA does not authorize to review supplement products for safety and effectiveness before they are marketed and sold.
Forrester Clinic, DBA Liposuction and Cosmetic Surgery Institute, Longevity Spa, and any other affiliates reserve the right to refuse fulfillment of any order for any reason. If an order is refused for any reason, and your credit card has already been charged, a full refund will be issued.
