Description
Pre-purchase Hollywood Laser Peel. Please fill in the ship-to and bill-to information to complete your order. Our staff will receive a notification detailing your order, and it will automatically be entered into your account in our practice management software indicating that you’ve prepaid for the treatment. Please keep a copy of your order confirmation. A physician will properly evaluate you prior to treatment. If it’s determined that you’re not a good candidate for the procedure, you can use the balance paid as a credit toward different procedures in our office. (If you’ve never visited our office before, no need to worry, we’ll enter you into our system based on the details you filled into the ship-to and bill-to fields and note your pre-purchase accordingly.)
